Appointments are scheduled at our Fishersville Office Monday through Friday between 8:00 a.m. and 4:30 p.m. or at our Lexington Office Monday through Friday (closed periodically, call to confirm office is open) between 9:00 a.m. and 4:00 p.m.
For your convenience, there are two ways you may request an appointment with us.

The Patient Portal will allow you to pre-register for your appointment by completing the required paperwork online, request future appointments, request prescription refills, and in the future receive some lab and test results at your convenience.
Please contact your pharmacy and they will submit an electronic request to your provider. New prescriptions will require the benefit of an appointment to evaluate your concerns. You may request an appointment or prescription refill through the Patient Portal.
