Insurance
It is extremely important that you are familiar with the requirements of your insurance plan prior to any scheduled visits to the office. Remember to bring your insurance card, photo ID, referral (if necessary) and co-payment, if required, to your appointment.
If your insurance company requires a referral from a Primary Care Physician (PCP), please contact your PCP prior to your appointment to obtain this referral. We may not be able to see you without the referral due to the regulations set by your insurance company. Under certain circumstances, our staff may not be able to obtain the referral for you at your appointment time.
Insurance Coordination for OB Patients
Our OB Coordinator will verify your insurance benefits and if there is an estimated out-of-pocket patient responsibility, she will establish a payment plan for you. We recommend you contact your insurance carrier prior to this visit to confirm your coverage.

If Your Insurance Changes
If you change insurance companies during the course of your treatment, please advise our staff of the new information. Insurance companies require claims to be filed by a certain date. If we do not receive this information, you will be responsible for your bill.
Payments & Fees
Payment is expected at the time any services are rendered. If you have any questions regarding billing, please call our business office at (540) 213-7750. We accept cash, checks, VISA, MasterCard and Discover.
Co-Pays & Balances
If your insurance company requires a standard co-payment for services, we will need to collect it on the day or your appointment in accordance with regulations set by your company. Please be prepared to pay the "patient due/balance" part of your bill at the time of your visit or, depending on your circumstances, we may be required to reschedule your appointment.
Cancellation Fees
To best serve all patients, we require a minimum of 24 hour notice to cancel or reschedule your appointment. In the event that the minimum 24 hour notice is not given, a $25 administrative fee will be charged and is not covered by insurance.
Insurance Coverage for Screenings
Your visit may include studies such as urinalysis, hemoglobin, etc. that are considered to be standard care. Many insurance companies to do not pay for these studies when they are performed for screening or preventive purposes. Payment for these services is not included in the standard co-payment amount and will be the responsibility of the patient. Please call your insurance company to find out your benefit coverage. You may be asked to sign a waiver for these services.
