APPOINTMENTS

Appointments are scheduled Monday through Friday between 8:00 a.m. and 5:00 p.m. by calling
(540) 213-7750 or (540) 942-7750.

We recommend that you arrive 10-15 minutes prior to your scheduled appointment to complete necessary patient information. For your convenience, you can download the forms from our site and have them ready when you arrive. Also remember to write down any specific questions you may have to ensure all of your concerns are addressed.

In order to best serve all patients, we ask that you give at least 24 hours notice if you will be unable to keep a scheduled appointment.

EMERGENCIES/AFTER HOURS

During regular office hours, call our office and the provider on-call will return your call promptly.

After hours, call the number listed below for your healthcare provider. Enter the phone number where you can be reached and keep the line open to allow the provider to promptly reach you. If you do not receive a call back within 20 minutes, call AMC Labor & Delivery at 332-4360 or 932-4360 and they can reach the provider on-call. If at anytime your symptoms or illness requires immediate medical intervention call 911 or go directly to the closest emergency room.

PAYMENT FOR SERVICES/INSURANCE

Payment is expected at the time any services are rendered. If you have any questions regarding billing, please call our business office at (540) 213-7750. Payment plans can be structured to meet your circumstances when necessary. We accept cash, checks, VISA, MasterCard and Discover.

It is extremely important that you be familiar with the requirements of your insurance plan prior to any scheduled visits to the office. Remember to bring your insurance card, photo I.D., referral (if necessary) and co-payment if required to your appointment.

AFFILIATED INSURANCE COMPANIES

Affiliated insurance medical companies include Medicaid, Medicare, Tricare, VA Premier, Optima, Anthem Blue Cross/Blue Shield, Southern Health, Cigna, First Health and United Health Care. If your insurance carrier is not listed, please call the office at 540-213-7750 for an updated list.

PRESCRIPTIONS

If you need authorization or a prescription refill, please call during office hours and allow 24 hours for processing. So that the on-call provider can better be available for emergencies, please do not request prescription refills on weekends, holidays or after hours.
